Club Sports Registration Forms

These forms are used for registering your Intramural team for a certain sport or league. Please make sure you fill out each field in order to submit the form. You will be contacted when the schedule is complete and the Captain's Meeting is approaching.

Sign up to play during the correct entry period. If the entry period is past, you will not be able to participate in that league/sport.
