This form is used, usually by course instructors, to report incidents of academic dishonesty. Once a course instructor and student meet to determine responsibility and penalty, if any, this form and supporting documentation are sent to the Associate Dean of the school in which the course is taught (Dr. Barbara J. Porter/College of Arts and Sciences; Dr. James M. McDonnell /School of Education and Human Services; Dr. James S. Valone /Wehle School of Business).
