2005 Dental Claims
Time is running out for submission of 2005 dental claims. Dental claim forms are available in the Human Resources Office (BA 108) and must be submitted to the Controller’s Office by March 15, 2006.
2005 Canisius Care Flexible Spending Accounts
The deadline is also fast approaching for 2005 Canisius Care claims. Claim forms are available in the Human Resources Office (BA 108) and must be submitted to the Controller’s Office by April 20, 2006. 2005 balances remaining after April 20, 2006 will be lost.
Understanding Your W-2 Form Wage and Tax Statement
Canisius W-2 forms for facilities management, public safety, clerical and technical staff were distributed with paychecks in January. All other W-2 forms were mailed on January 25, 2006 to the person’s home. The W-2 is your wage and tax statement and must be included when filing your tax return.
