Understanding Your W-2 Form Wage and Tax Statement
Canisius W-2 forms for Maintenance, Public Safety, Clerical and Technical were distributed with paychecks in January.  All other W-2 forms were mailed on January 25, 2005 to the person’s home.  The W-2 is your wage and tax statement and must be included when filing your tax return.

2004 Dental and Canisius Care Flexible Spending Accounts
Time is running out for submission of 2004 Canisius Care and Dental claims.  Dependent, health care spending account and dental claim forms are available in the Human Resources Office (BA 108) and must be submitted to the Controller’s Office by March 15, 2005.  2004 balances remaining after March 2005 will be lost.
