Understanding Your W-2 Form Wage and Tax Statement
Canisius W-2 forms for maintenance, public safety and clerical and technical personnel were distributed with paychecks in January.  All other W-2 forms were mailed on January 25, 2005 to the person’s home.  The W-2 is your wage and tax statement and must be included when filing your tax return.  To help you better understand the W-2, the Human Resources Office has prepared a list of frequently asked questions (FAQs) and answers.  Click here to read the W-2 FAQs.

2004 Dental and Canisius Care Flexible Spending Accounts
Time is running out for submission of 2004 Canisius Care claims and dental claims.  Dependent, health care spending account and dental claim forms are available in the Human Resources Office (BA 108) and must be submitted to the Controller’s Office by March 20, 2005.  Balances from 2004 that remain after March 2005 will be lost.

Leave the driving and parking to NFTA
Did you know that Canisius College offers a tax savings program for NFTA commuters?  The program currently has a membership of 10.  All purchases are made on a pre-tax basis – providing another savings for those using the NFTA.  Faculty and staff may purchase a monthly flash-a-pass for $55.00, suburban flash-a-pass for $66.00 or 20 tokens for $27.00.

How does it work?  Orders must be placed by the 15th of the month for passes effective the first of the following month.  Purchases will be made with pre-tax dollars simply by completing a pre-tax election form.  If you are interested in participating, please call Jennifer in Human Resources at extension 2247 by the 15th of the month.  The cost will be deducted from your next paycheck.

Open Enrollment for Health Insurance
Human Resources will conduct the annual open enrollment meetings for health insurance on Wednesday, April 27, 2005 in Churchill 204.  Watch your mailboxes for more information regarding these meetings.
