COBRA Continuation Coverage
Eligible dependents covered by your Canisius College family health insurance plan may continue coverage upon your death, divorce, or entitlement to Medicare. To qualify for continued coverage, dependents must:
- Be covered in a college health insurance plan at the time of the event
- Notify the Human Resources Office within 60 days of the event
- Submit a completed election form to the Human Resources Office
- Pay the full cost of coverage
The same notification and election procedure applies to dependent children who lost eligibility for coverage under a family plan, primarily by “aging out” (passing the birthday at which coverage ends) or no longer being enrolled as a full-time college student. Please note it is your responsibility to initiate COBRA continuation coverage. Information and COBRA election forms for college-sponsored health insurance plans are available in the Human Resources Office, Bagen Hall, Room 108.
