Students who need verification of enrollment for any reason should follow one of the following steps:
- Complete the Enrollment Verification Request Form in person at the Student Records Office.
- Download the Enrollment Verification Request Form (.pdf) and submit the completed form to the Student Records Office in person, by mail or by fax.
