Canisius College

• Complete the Application for Graduation (adobe Acrobat 11.8 KB).
• Fill out the form with all your information online.
• Print the completed document and return to Student Records Office, Bagen 106.
• If you need a replacement diploma because your original was lost or damaged, download and submit the Replacement Diploma Request form (Adobe Acrobat 12.0 KB) with the $30.00 replacement fee.  Be sure to include the degree and date the degree was awarded.  Make checks payable to Canisius College. It takes approximately two weeks to receive the diploma.  Please note that all replacement diplomas have a re-issue date printed in the lower left hand corner.  Send the request for the replacement diploma to the Student Records Office at the address below.
