Physical Examination Requirement for New Undergraduate and Transfer Students ( United State and Canadian Residents)
All full time undergraduate and full time undergraduate transfer students are required to submit a completed physical examination and immunization form prior to their first class attendance. Physical examinations must be done within one year of attendance. Students playing a D1 Intercollegiate sport must have a physical done within 6 months of first sports participation. Physical examinations must done within one year of attendance. Students playing a D1 Intercollegiate sport must have a physical done within 6 months of first sports participation.
Physical examinations for students attending in the fall semester must be submitted to Student Health by July 31. Students attending in the spring semester must submit physical examination form by January 1.
Physical Examination Services in Student Health
The Student Health Center offers physical examinations for class attendance. To schedule an appointment for a physical, please call the Student Health Center at 888-2610. Students will be charged $100. Payment is required at the time of service. Physical examinations can also be obtained through your health care provider, urgent care clinics, travel clinics and county health departments.
