Time Sheet (MS Word Version) (PDF Version): The student fills out the time sheet and has it signed by the site supervisor each time he/she participates at your site.
Placement Site Assessment (MS Word Version) (PDF Version): After the form has been signed and the student's performance has been assessed, the student gives the form to his/her professor. After the professor has seen this form it may be returned to the student for his/her portfolio.
Service-Learning Information/ Liability Form (MS Word):This form is submitted by the student before beginning service in the community.
The Service-Learning Student Evaluation (MS Word): This is an evaluation for the students to describe and rate their service-learning experiences at Canisius College.
Service-Learning Pre-Experience Survey (MS Word) Students complete a survey before their Service-Learning experience.
Service-Learning Post-Experience Survey (MS Word) After students have completed their service experience, students submit a brief survey to evaluate their experience.
