- Reflection Suggestions: This information provides a guide for reflecting on the service-learning project.
- Service-Learning Pre Experience-Survey
A brief survey to evaluate before your service experience. - Service-Learning Information/ Liability Form: This form is to be read, the required information entered and submitted before beginning your service.
- The following form is to be printed and given to your professor at the end of your service experience:
- Time Sheet (MS Word Version) (PDF Version): Fill out the time sheet and have it signed by the site supervisor each time you participate in your service.
This form is to be printed and given to the person at your service site who is familiar with the service you rendered.
Service-Learning Student Evaluation: This is an evaluation for the students to describe and rate their service-learning experiences at Canisius College.
Please fill out the following form after you have completed your service:
- Placement Site Assessment (MS Word Version) (PDF Version): Give the form to your professor after it is signed. After your professor has seen this form you may keep it for your portfolio.
