Insurance Information

Canisius College strongly recommends that all students have some type of health insurance.  Students without health insurance may put off seeking appropriate medical treatment for illnesses or injuries fearing financial burden. This can lead to great physical and emotional discomfort for the student.  In addition, the financial burden associated with unforeseen medical expenses for an uninsured student may force the student to withdrawal from school and interrupt academic plans.

Health Insurance for Domestic Students
M and T Insurance Agency offers a student sickness and accident plan that provides basic coverage for medical and accidental injury expenses. Canadian students are eligible to enroll in this plan.

Enrollment - On Line Option
To enroll on line, using a major credit card,  log onto www.ajfusa.com. Select Tools for Students/Participants and Policy Holders, then select College and University Students. You will be directed to a page with a drop down box, select Canisius College ( domestic). 

Enrollment - Mail Option
Students can download the enrollment form here (Adobe Acrobat PDF) and send it with a check to the Allen J Flood Company.

Health Insurance Cards
Insurance Cards are available for download here (Adobe Acrobat .pdf).  Cards can also be printed from www.ajfusa.com. Student Health does not distribute insurance cards.

Deadlines for Enrollment
Students wishing to purchase the basic sickness and accident plan must enroll by the following dates: 

September 30, 2010 for  fall and annual plans
February 7, 2011 for spring/summer plan

Enrollment is closed after these dates.  However, if you have had coverage through a parent or employer and this coverage terminates, you  can purchase the College plan.  To do so, submit proof of termination of previous coverage, along with the enrollment form and check to: The Allen J. Flood Companies, Inc., Two Madison Avenue, Larchmont, New York 10538

Filing A Claim
To file a claim you must submit  itemized medical bills from the physician, hospital, clinic, etc. along with a claim form, to Klais and Company, 1867 West Market Street, Akron, Ohio 44313. Claims must be submitted no later than 90 days after the injury or illness has occurred in order to be considered for payment.

Claim Forms
Claim forms are available here for the 2010-2011 plan.  Claim forms for the 2009-2010 plan year can be picked up in Student Health. 

Student Health recommends that you make a copy of all claim forms and bills before submitting them to the claims administrators. Students can also turn completed claim forms and bills into Student Health. Student Health will forward them onto M and T Insurance Agency for processing.

Claims Problems
If you experience a delay in payment or have questions about your claim, contact Klais and Company at 1-800-331-1096 .  If you need additional assistance, please call M and T Insurance Agency at 716-853-7960, ext. 4220. A representative is also be on campus twice a month in the Student Health Center. To set up an on campus appointment,  call 716-888-2610.

Accident and Accidental Death Benefit
Accident: 
All full-time undergraduate students are covered under a student accident plan. 

This plan will pay for medical expenses associated with an accident, both on campus and off campus accidents, up to $3,000.  This coverage is considered excess coverage, which means students who have other health insurance, should submit to their primary plan first.  Any unpaid expenses after review by the student's plan can be submitted for payment to Klais and Company.  Students who do not have any other insurance should submit all bills to Klais and Company following the steps under the filing a claim section above.

Accidental Death Benefit:
All full-time undergraduates are covered for a $5,000 benefit for death resulting from injuries and/or illness associated with an accident.  Family members should contact M and T Insurance Agency at  716-853-7960, Ext 4220 to discuss this benefit.

Travel Assistance Benefit
The Student Sickness and Accident plan for 2010-2011 provides travel assistance services. Travel assistance benefits are outlined in the brochure.

Unisured Students with Limited Income
Students who are New York State residents, with limited income, may be eligible for health benefits offered through New York State. Visit the New York State web site at www.health.state.ny.us, click on info for consumers to learn about Family Health Plus and Medicaid.  Medicaid also has a helpline number at (518-486-9057).
