Welcome to Canisius College and the Student Health Center. We look forward to meeting you at orientation and providing you with quality health care during your studies at the college.
Canisius College has very specific health requirements for new students. Taking time to address these requirements before August 1, 2009 will make your first month at school less challenging. Students who do not comply with these health requirements will have a registration hold placed and if a resident student, removed from college housing for non compliance, thirty days after the start of the fall semester. Below is a list of those requirements and a time line for completing them.
Physical Exam RequirementAll members of the Class of 2013 are required to have a complete physical examination done by their doctor, after March 1, 2009 and submitted to Student Health no later than July 31, 2009. A physical examination form will be mailed to you in early March.
Canisius College will not accept any other physical examination form. You must take the college form to your doctor’s appointment. Do not download the form that is currently on this web site now, since it is the form for the 2008-2009 academic year and will not be accepted.
Before you mail the form back to the college, check it over carefully, make sure all sections are complete and that requested signatures and dates are provided. Incomplete physicals will not be accepted.
ImmunizationsMeasles, Mumps and RubellaAll members of the Class of 2013 are required by New York State Law to submit proof of immunizations against measles, mumps, and rubella .The immunization section of the physical examination form explains those requirements. Be sure your doctor completes the immunization section of the form and signs his/her name at the bottom of that section.
Advise your doctor that the college does not accept any attached immunization records or other vaccination documents.
Meningitis
All newly accepted students are required to read about the risk of meningitis on college campuses and submit the date they received either menomune or menactra vaccination.
If you decide you do not want the vaccination, you must sign off that you are aware of the risks and decline vaccination. This is a New York State mandate. If you are a minor, your parent or guardian must sign for you.
Tuberculosis Screening
All students are required to be screened for tuberculosis by their health care provider.
Be sure your provider completes the TB section of the form and submits results of the screen. If your form is missing a completed TB screen, it will be deemed incomplete and not accepted.
Health InsuranceIf you have health insurance, request a copy of the card from your insurance company to carry with you at school. Please send a paper copy of the card to Student Health along with your physical. Check with your insurance company now to find out how your plan works while you are in college.
If you do not have health insurance, the college offers a sickness and accident plan.
Information about the 2009-2010 plan, including cost and benefits, will be available on-line site July 1, 2009. Canisius strongly recommends that all students have health insurance coverage.
Class of 2013 Timeline for Health Requirements
March 2009
- Download your physical examination form if you have not received one in the mail.
- Make an appointment with your doctor sometime after March 1, 2009 and before July 31, 2009 for a complete physical examination.
- Contact your health insurance carrier to request your own health insurance card. Ask member services how your plan works while you are a college student.
April 2009
- Discuss medications you take everyday with the doctor who prescribes them. Make sure you bring enough medication with you to campus to last until your next visit home or have prescriptions sent to a pharmacy near campus. A list of pharmacies is found on the Student Health Center web site under services.
- Arrange to see any medical specialists (dermatologist, orthopedist counselor,dentist etc) before August 2009 and make follow up appointments over breaks in October, November or December when you will be back home.
June 2009
- If you need additional services on campus such as counseling or disability services, stop into those offices during orientation.
July 2009
- Check over your physical examination form. Make sure each section has been completed, signed,and dated as requested.
- Mail your physical examination form to Student Health by July 29, 2009
August 2009
- Prepare a first aid bag for your dorm room including a thermometer, cold medications, acetaminophen or ibuprofen, first aid supplies such as Band aids, antibiotic ointment, ice pack, and ace wrap.
- Purchase cleaning supplies for your dorm room and pack a supply of alcohol based hand sanitizer.
