

All full time undergraduates, including transfer students, and all international graduate and undergraduate students are required to submit a completed physical examination and medical history form prior to their first class attendance. Physical examinations must done and dated after March 1, 2008 for fall 2008 attendance and after August 1, 2008 for January 2009 class attendance.
Student Health strongly recommends that all students use the college physical examination form. Forms from your doctor's office, another school, a military physical or other form including a copy of an electronic medical record of a physical examination may not meet the requirements of the college and will be determined incomplete and not accepted.
Physical examination and medical history forms are mailed to all newly deposited students. Additional copies are available for download here (Adobe Acrobat .pdf). International students must use a special physical examination form designed specifically for them. This form is available in International Student Programs.
Transfer students can submit a copy of a recent physical examination and medical history form from a previously attended institution of higher education if the physical was done within 6 months of attending Canisius. Canisius may require more medical information or a new physical examination if the information provided is not current, incomplete or does not otherwise comply fully with the PE requirement in place for all other Canisius students . Transfer students should contact Student Health to check compliance with Canisius requirements after submission of their health information.
Deadline for submission of the physical examination form for fall 2008 attendance is July 31, 2008 and January 1, 2009 for spring 2009 attendance. Students will be notified by college e-mail if they have not submitted a physical examination form by these deadlines.
Students who do not submit the physical examination form will have a registration hold placed thirty days after the start of first semester at Canisius preventing further registration at the college. Academic deans and faculty are notified of non compliant students. Students will be notified of a registration hold via college e-mail. A letter will be sent home to parents of non compliant undergraduate students advising them of the registration hold.
The Student Health Center does offer physical examinations and immunizations needed for class attendance. To schedule an appointment for these services please call the Student Health Center at 888-2610. Students will be charged a fee for these services. Payment is required at the time of service. Physical examinations cost $100 in the Health Center.
