Initial Appointment

Before the initial appointment, students are asked to complete an intake form, which collects demographic information, personal and family history, along with reasons for accessing counseling services. The student then meets with one of our counselors to clarify concerns and to assess their needs. 

All counseling sessions are free, private, and confidential. Only with a student’s written permission are we able to share information with anyone. Counseling records do not become part of a student’s academic record.  Counseling records are private and confidential. Refer to “Our Commitment to Confidentiality” below.

Confidentiality: Our Commitment to Confidentiality

Counseling sessions and counseling records are confidential. Student privacy is important to us and is protected by state laws and professional ethical standards. No information about counseling appointments will be released without a student’s written authorization. However, confidentiality is legally limited. Those limitations include: if it becomes apparent that someone may do harm to self or to others, if information is subpoenaed by court and in the case of mandatory reporting of child abuse/neglect.
