If you have been provided a record check form by a third party, please follow the steps below:
Complete the student section of the third-party form in full.
Sign the form, if required, to authorize Canisius University to release your student conduct record.
Submit a request for a Student Conduct Record Check to the Office of Student Conduct, attaching the completed and signed documentation.
Requests will be processed in accordance with Canisius University policy and applicable federal privacy laws (FERPA). Processing times may vary depending on the nature of the request and the volume of submissions.
Third Parties: Initiate a Record Check
Third parties may request a Student Conduct Record Check from the Canisius University Office of Student Conduct. All requests must be submitted in writing and include the following information:
