Immunization and Health Requirements

All incoming undergraduate, athletes, and all international students are required to submit a completed physical examination and immunization form prior to class attendance. Graduate students must refer to instructions listed under New York State Immunization Requirements below.

Step 1. Print the Physical and Immunization Form. Complete all of page 1, and write your name at the top of pages 2-4.

Step 2. Schedule an appointment with your primary care provider, and bring the form with you to be completed and signed. 

Step 3.  Submit your completed form to Student Health by July 31st if attending in the fall, and January 1st if attending in the spring. Students who have been accepted after these dates must submit their documents as soon as possible, and no later than 30 days after the start of the semester.

International Students

International students from certain high-risk countries are required to be screened for tuberculosis upon arrival at the college. The Student Health Center can provide this screening with a blood test.   A history of BCG vaccine not meet this requirement. The TB screening requirement is strictly enforced by the college.

All health forms and test results must be in English. Forms submitted in another language will not be accepted. International students can contact the International Institute to arrange for their physical examination form to be translated into English. There is a fee for that service. 

New York State Immunization Requirements

All incoming students enrolled in 6 or more credits on-campus are required by New York State to comply with Public Health Laws 2165 and 2167. These laws apply to all students, including undergraduates, graduates, commuters, residents, US citizens, international, undergraduate and graduate summer students, cross registered, and graduate students in online programs with residency requirements or a “modified online” format.  

Graduate students do not need to submit a physical examination, but are required to submit proof of vaccinations using the Graduate Students and Part-time Students Immunization Form.

Canisius University strictly enforces compliance with these laws.  Students who do not submit the required information within 30 days of the first day of class will have their registration withdrawn. Withdrawn students cannot attend class, reside in college housing, access college technology, participate in or attend campus events. 

All applicable dates for tuition refund will be applied to immunization withdrawal. If withdrawn, there is a $300 fee to re-register once compliance requirements are met.

Measles, Mumps and Rubella (New York State Public Health Law 2165)

•    2 vaccinations against measles and 1 vaccination for mumps and rubella are required
•    The immunizations must have been given on or after your first birthday, and at least 28 days apart.
•    Your doctor's office or your high school should have your immunization records. If you are unable to locate immunization records, your doctor can order blood testing to prove your immunity.
 
Meningococcal Meningitis (New York State Public Health Law 2167)

The requirement is one dose of meningococcal vaccine against strains ACYW within the past 5 years, and/or proof of a completed two or three dose vaccination series for meningococcal stain B within past 5 years.

If you or your parent/guardian (if you are under 18 years of age) do not wish vaccination against meningococcal disease, you or parent/guardian must sign the declination statement on page 4 of the Physical and Immunization form.
